Jacksonville public health clinics, WIC, vaccines and environmental help
For Jacksonville, Richlands, Swansboro, Holly Ridge, Sneads Ferry, Hubert and surrounding Onslow County communities, the right public-health contact depends on what you need. Clinical care and WIC start at one set of offices; septic, wells, restaurant permits and inspections use Environmental Health at a different address.
Use this 2026 guide to call the right office, prepare for an appointment, request the correct record, understand sliding fees and avoid an unnecessary drive across Jacksonville.

How to make an Onslow County Health Department appointment
Call 910-347-2154 for a Health Department appointment. The county also publishes an online appointment-request form, but calling remains the clearest route when you need to confirm whether a service is appointment-only, accepts walk-ins or requires specific documents.
Say “immunization,” “child physical,” “family planning,” “STI testing,” “TB screening” or “international travel clinic” instead of asking generally for an appointment.
Some programs accept walk-ins when staff and clinic capacity allow, but that does not guarantee same-day service.
Depending on the service, you may need photo ID, insurance cards, proof of income, immunization records, school forms or previous medical information.
Ask whether the service uses the sliding fee scale, insurance billing, a fixed charge or a state-funded program.
Clinical appointments usually route to College Street. Environmental Health applications route to NW Corridor Boulevard.
Child health, reproductive health, STI testing, TB and travel services
Well-child physicals are available through age 20, including daycare, pre-K, kindergarten, school and sports physicals. The county specifically notes that Child Health does not provide ordinary “sick visit” care.
Services include breast exams, cervical cancer screening, pregnancy testing, physical exams, family-planning services and prenatal-clinic care.
The clinic offers confidential STI/HIV testing, evaluation, counseling and treatment for many STIs. Scheduled appointments and walk-ins are accepted based on availability.
TB risk assessments and screenings are generally available Monday-Wednesday and Friday. The county currently lists a $30 charge when insurance does not cover the assessment or screening.
The travel program reviews trip plans and may recommend destination-specific vaccines and preventive health steps. Call before travel rather than waiting until the final week.
The department provides pregnancy and early-childhood care-management programs plus community referrals and public-health education.
Vaccines, shot records and what to bring
Onslow County’s Vaccine Clinic provides routine childhood vaccines, many adult vaccines and selected travel vaccines. The department records administered vaccines in the North Carolina Immunization Registry, which helps participating providers and schools access statewide immunization information.
The Vaccines for Children program provides state-funded vaccines for qualifying children ages 0 through 18. Bring the child’s shot record, insurance information if insured and the requested identifying information.
Available services include vaccines such as influenza, Tdap/Td, shingles, pneumonia, hepatitis combinations and other recommended vaccines depending on supply and eligibility.
The county currently lists travel options including yellow fever, typhoid and Japanese encephalitis. Availability should be confirmed before relying on the list.
Onslow County WIC offices, phone and eligibility basics

Pregnant, breastfeeding or postpartum women, infants and children under five who meet program requirements.
The county says applicants already receiving Medicaid, TANF or Food and Nutrition Services are automatically income-eligible for WIC.
Call 910-347-5002 and choose option 2 for the WIC breastfeeding-support warm line.
Medical records are different from birth and death certificates
Onslow County has several record systems. The easiest mistake is asking the Health Department for a certified vital record or asking the Register of Deeds for a clinic chart.
Use the Health Department medical-record request process for records created by OCHD programs such as immunizations, family planning, maternity, STI, TB or child-health care.
Use the Onslow County Register of Deeds, located at 602 Anne Street in Jacksonville, phone 910-347-3451.
Environmental Health public-record requests use the county records process rather than the clinical medical-record workflow.
Requesting your OCHD medical record
The patient right-of-access form currently linked by the county lists medical-record fax 910-347-0728 and email ochd-medicalrecords@onslowcountync.gov. The form allows delivery by mail, email, fax, patient portal or in-person pickup where applicable and says processing may take up to 30 days.
Patient Portal
The Onslow County patient portal is for patients age 18 or older. The county says staff begin registration during an office visit and a signed consent form is required before access is activated. Portal features include test results, billing statements, demographics, listed allergies and medications.
Septic, wells, restaurants, lodging and environmental permits
Environmental Health works from 234 NW Corridor Boulevard, Jacksonville, NC 28540, not the College Street clinic. The main number is 910-938-5851, and regular hours are Monday-Friday, 8 AM-5 PM.
Soil evaluations, new septic systems, existing systems, repair permits and onsite wastewater approvals.
Well applications and environmental-health requirements for regulated private drinking-water wells.
Restaurants, food-service establishments, temporary food operations, lodging, institutions and related sanitation inspections.
Environmental Health Specialists inspect sanitation conditions at regulated child-care facilities.
Application and environmental-health inspection requirements apply to regulated tattoo operations.
Use the county’s “What’s the Grade?” system to search current restaurant and lodging inspection information.
How the Onslow County septic permit process works
For undeveloped land or a project that changes the permitted septic capacity, Onslow County uses a three-stage permitting process. The practical sequence is important because later construction approval depends on the earlier environmental review.
Submit the onsite wastewater application and property map or site plan. If someone other than the owner applies, owner authorization is required.
Show the proposed structure, driveway, water supply and septic area, and make property corners identifiable in the field so the evaluator can complete the site visit.
If the site is suitable, the first permit identifies the approved septic-system type and permitted bedroom capacity.
The second stage authorizes installation and is needed before the septic system can be installed and before certain building-permit steps move forward.
The final permit is issued after installation inspections have been completed and the system has been approved.
Do not repair a failing system before obtaining the required Health Department repair permit. The current county repair page says there is no fee for that repair-permit service at this time.
If bedrooms are not increasing and the addition will not encroach on the septic system or well, an Existing System Authorization may be the correct route. The current county page lists a $150 application fee.
An increase in bedroom count or a system relocation will generally require an Improvement Permit and Construction Authorization. Confirm with Environmental Health before designing the project.
Restaurant permits, food inspections and temporary events
Food-service establishments that prepare or serve food to the public for payment must obtain the applicable Environmental Health permit before operating. Onslow County also inspects permitted establishments and posts sanitation grades.
Contact Environmental Health before opening. Depending on the operation, plan review, an application and pre-opening compliance steps may apply.
Temporary food vendors use a separate application. Filing deadlines and fees can change, so use the current form linked by Environmental Health rather than an old downloaded copy.
Onslow County provides an interactive “What’s the Grade?” lookup for newer restaurant and lodging inspection records.
Sliding fees, insurance and paying an OCHD bill
Certain Health Department clinical services use a sliding fee scale. Staff determine the charge using household income and household size, so patients should bring proof of income when requested.
The current billing page says clients have 30 days to provide proof of income. If it is not received, the client is billed at 100% of the applicable charge.
The county states that a client is not refused services simply because income documentation is unavailable when presenting for care.
The current list includes Blue Cross Blue Shield, Medicaid/PHPs, Aetna, Cigna, Tricare, United Healthcare/UMR, MedCost and limited Medicare Part B coverage.
The Health Department accepts cash, checks and major credit cards for medical bills. For phone payment or billing questions, call 910-347-2154 and ask for billing.
After-hours public health and Environmental Health reporting
For an immediate medical emergency or life-threatening situation, call 911. The routine Health Department appointment line is not emergency medical care.
Healthcare providers needing to report a reportable disease after normal hours are directed to call Onslow Memorial Hospital at 910-577-2345 and ask the operator for the Onslow County Health Department 24/7 call-down list.
The county’s reporting page directs operators of permitted food, lodging, pool, child-care and institution facilities to 910-455-0122 after hours for incidents such as fire, utility outage, pool accident or sewage backup.
The current Environmental Health staff directory separately labels 910-548-3765 as an “After Hours Emergency Phone.” During business hours, call 910-938-5851 if you are unsure which after-hours route applies.
